| Blue Shield 65 Plus is a Medicare-contracted HMO with a Medicare+Choice contract open to all Medicare beneficiaries entitled to Medicare Parts A and B. Beneficiaries without Part A must purchase coverage from Social Security. Members must pay Part B premiums and use Blue Shield 65 Plus providers for routine care. Minimal copayments apply to some services. Plan premiums apply in some counties. Brand name prescription drugs are limited and must be included on Blue Shield's comprehensive formulary. The formulary may change at any time without notice. |
